Community Health and Dental Care is seeking a Certified Registered Nurse Practitioner to join our on-site team in Pottstown, PA. This full-time exempt role provides medical evaluation, treatment, and education under medical protocols, collaborating with the Chief Clinical Officer.

The position requires 3–5 years in Family Practice, PA NP certification, and BCLS; DEA number preferred and ACLS preferred. Competitive base salary and comprehensive benefits are offered.

Qualifications

• 3–5 years’ experience in Family Practice, highly preferred.
• Current Nurse Practitioner Certification in Pennsylvania.
• Certification as a Nurse Practitioner by a nationally accredited nursing organization.
• DEA Number preferred.
• BCLS required; ACLS preferred.

Responsibilities

• Provides medical evaluation, treatment, and consultation to patients.
• Delivers care under medical protocols and coordinates with leadership.
• Documents patient history, exam findings, and treatment plans.
• Orders refills, labs, referrals, and coordinates follow-up.
• Participates in on-call schedule and patient education.
• Ensures timely completion of documentation for insurance/reimbursement.

About the Certified Registered Nurse Practitioner Position

Responsible for coordination & quality assurance of clinical programmatic & operational activities including diagnosis & treatment of illness, & patient education under medical protocols. Provides professional medical services to complement the delivery of primary health care services, Works closely with the Chief Medical Officer and Chief Clinical Officer.

Duties include but are not limited to:

Clinical:

• Provides medical evaluation, treatment, and consultation services to patients of the health center.
• Responds to emergencies and answers medical questions.
• Obtains and documents, or causes to be obtained and documented, appropriate chief complaint, past medical, family, and social history, review of systems, examination, medications and allergies, assessment, and plan for each patient.
• Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals.
• Reviews ancillary test results and coordinates notification to the patient.
• Provides patient education as needed.
• Supervises patient care by non-health center agencies such as home health care, nursing home, or hospice.
• Available to patients personally or through clinical staff to answer questions and relay information regarding their care.
• Completion of appropriate paperwork/computer documentation to comply with insurance and reimbursement guidelines.
• Completion of all documentation and paperwork in a timely manner.
• Participates in on-call schedule.

Medical Administrative:

• Participates in public relations and community outreach activities, as center shall from time-to-time request.
• Provides assistance with OSHA, emergency preparedness, infection control and regulatory compliance.

• Adheres to the organization’s policies regarding time and attendance.
• Comply with accepted dress code and maintain a professional image.
• Demonstrates reliability and trustworthiness.

Education and/or experience

• 3-5 years’ experience in Family Practice highly preferred
• Current Nurse Practitioner Certification in Pennsylvania

• ACLS preferred.
• Attendance required at assigned CHDC site(s)

Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.
