Qualifications

• LPN license required by state and current.
• One year LPN experience in last five years preferred.
• Excellent communication skills (verbal and written).
• Ability to work daytime hours; flexibility may be required to meet staffing needs.

Responsibilities

• Provides direct patient care within scope of practice for pediatric, adult, and geriatric patients.
• Conducts focused assessments and reinforces patient education.
• Administers medications, immunizations, and treatments such as injections, wound care, catheter care, and suture removal.
• Supports population needs for preventive care services, chronic disease management, and quality improvement initiatives.
• Assists with in-clinic procedures.
• Independently manages a nurse visit calendar and clinic workflows to support timely, coordinated care.
• Demonstrates strong communication skills and may assist with mentoring, instruction, committees, or quality improvement projects.
