Your responsibilities will include completing health assessments, training staff, and providing phone-based support during weekends. Candidates should possess a valid LPN or RN license with managerial experience in healthcare.

Qualifications

• Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN); BSN preferred.
• 2+ years of managerial experience.
• Experience with ID/DD population preferred.
• Proof of valid Indiana nursing license.
• Valid Driver’s License.
• Current auto insurance.

Responsibilities

• Ensure individuals and staff are appropriately trained and educated.
• Work with other LPNs to ensure health and well-being of individuals.
• Complete all physical assessments as needed.
• Attend quarterlies/annuals and ensure documentation is organized.
• Teach Core A and B.
• Field phone calls for individual triage.
