Qualifications

• Must hold a current LPN or RN license (Indiana).
• BSN preferred; 2+ years managerial experience.
• Experience with ID/DD population preferred.
• Valid driver’s license and auto insurance.

Responsibilities

• Coordinate training and education of staff on care procedures.
• Lead and supervise Medical Assistants and nursing staff.
• Conduct physical assessments and document wellness reports.
• Manage risk plans, medical history, and medication administration.
• Attend meetings and coordinate with care teams and families.
