Job summary

Hood River Post Acute Hood River SNF Healthcare, LLC is seeking a dedicated LPN supervisor to direct daily nursing assistant activities and oversee shift operations in our long-term care facility in Hood River, Oregon.

You will ensure adherence to nursing policies, participate in QA and surveys, manage documentation, and coordinate care plans, while leading staff and maintaining a warm, compliant care environment for residents.

Qualifications

• Must possess a Nursing Degree or be a graduate of an approved LPN program.
• Current, unencumbered LPN license in the state.
• Current CPR certification.
• Experience in long-term care nursing desirable.
• Strong leadership and supervisory abilities.

Responsibilities

• Direct the day-to-day functions of nursing assistants and plan shifts.
• Maintain Nursing Service Procedures Manual and QA program.
• Complete patient records, charts, and discharge planning as required.
• Lead staffing, conduct rounds, and evaluate performance.
• Ensure compliance with policies and HIPAA regulations.
