Job summary

United Church Homes seeks an attentive Staff Nurse (LPN) for Trinity Community at Beavercreek in Ohio. The LPN will coordinate care for about 25 residents, perform medication administration, monitor vitals, and collaborate with RN staff to ensure quality care and safety across shifts.

Responsibilities include completing nursing assessments, following care plans, documenting changes, and assisting with admissions, discharges, and transfers as needed. CPR and an active LPN license are required.

Qualifications

• Successful completion of a state-approved pre-licensure nursing education program is required.
• One year of nursing and/or long-term care experience preferred.
• Active LPN license in good standing; CPR certification.
• Must obtain and maintain mandatory state and federal requirements and certifications.

Responsibilities

• Manages resident treatments and care delivery within scope of practice.
• Distributes and administers prescribed medications and treatments.
• Assesses residents and contributes to nursing assessments and care plans.
• Documents procedures, medications, and patient responses accurately.
• Coordinates and participates in care conferences and education for residents and families.
