Job summary

United Church Homes — Trinity Community at Fairborn is seeking an experienced Staff Nurse (LPN) for the 6a-6p shift. You will coordinate care for about 25 residents, administer medications, and perform nursing assessments to ensure quality care and safety.

Responsibilities include rounds, documentation, collaboration with physicians and family, and adherence to care plans and protocols. LPN licensure, CPR, and strong communication are required for this role.

Qualifications

• Successful completion of a state-approved pre-licensure nursing education program.
• One year of experience in nursing and/or long-term care is preferred.
• Active LPN license conferred by the State Board of Nursing; CPR certification.
• Ability to read, write, understand and speak the English language.
• Adhere to the United Church Homes Employee Handbook and safety protocols.

Responsibilities

• Manages resident treatments and coordinates care delivery.
• Distributes and administers prescribed medications and treatments per protocols.
• Assess residents under RN supervision and participate in care planning.
• Document and communicate resident changes to supervisor and physician.
• Oversee adherence to safety, infection control, and dietary considerations.
