Job summary

The Trinity Community at Fairborn, operated by United Church Homes, is seeking an experienced Staff Nurse (LPN) to coordinate care for about 25 residents per shift. You will administer medications, perform assessments, and collaborate with RNs and physicians to implement care plans in accordance with policies and regulations.

Ideal candidates hold a state-approved LPN license, CPR certification, and have solid geriatrics experience.

Qualifications

• State-approved pre-licensure nursing education program must be completed.
• Active LPN license in the state and CPR certification are required.
• Experience in long-term care or geriatrics is preferred.

Responsibilities

• Manages resident treatments and coordinates care for about 25 residents.
• Distributes and administers prescribed medications and IVs if certified.
• Performs nursing assessments and participates in care planning.
• Documents treatments, observations and responses per policy.
• Oversees admission, discharge, and transfer processes and coordinates with other departments.
