Job summary

UnityPoint Health in Des Moines, IA is seeking an LPN to join our nursing team on a floor caring for diverse medical and surgical patients. Flexible Full-Time or Part-Time Day shifts from 7am to 7:30pm with every third weekend and holiday rotation.

You will deliver direct patient care and support a culture of learning and high-quality outcomes. The role requires completion of an LPN program, Iowa licensure, and BLS.

Qualifications

• Completion of a licensed practical nurse program from an accredited college.
• Current licensure in good standing to practice as an LPN in Iowa.
• Must possess and maintain BLS certification or obtain within three months from hire date.
• Preferred experience in Post-Acute or Acute Care settings.

Responsibilities

• Provide hands-on nursing care using the nursing process to achieve quality outcomes.
• Educate patients and families and create individualized discharge plans.
• Coordinate nursing activities with the rehabilitation team to meet goals.
• Supervise and direct unlicensed assistive personnel as appropriate.
• Assess physical, psychological, sociocultural, and spiritual needs of patients and families.

Responsibilities

• Possesses the knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness
• Participates in patient/family educational activities and uses appropriate resources to implement an individualized teaching and discharge plan with patients and their families
• Performs hands‑on nursing care by utilizing the nursing process to achieve quality outcomes for patients
• Provides direction and supervision of unlicensed assistive personnel (i.e. Patient Care Technician), demonstrates clinical judgment appropriate for Iowa’s LPN scope of practice
• Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals
• Contributes to the assessment of the physical, psychological, sociocultural, and spiritual dimensions of patients and their families, as well as their educational and discharge needs
• Implements the interdisciplinary plan of care within the scope of LPN/LVN practice and provides nursing care and education directly or through ancillary personnel, as needed, to maintain and restore function and prevent complications and further loss
• Contributes to the evaluation of the nursing care and to the modification of the plan as needed to achieve measurable goals and objectives

Qualifications

Education

• Successful completion of a licensed practical nurse program from an accredited college.

License(s)/Certification(s)

• Current licensure in good standing to practice as a Licensed Practical Nurse in Iowa.
• Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.

Experience

• Preferred experience or clinical time in Post-Acute Care or an Acute Care environment.

Other

• Level 3 Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
