Qualifications

• High school diploma or better required.
• Experience in long-term care preferred.
• Licensed Practical Nurse (LPN) required.

Responsibilities

• Utilize nursing knowledge for safe basic care aligned with standards.
• Administer medications accurately with documentation of patient response.
• Coordinate communication among patient, family, staff and physicians.
• Assist with shift change reports and ensure pertinent information is relayed.
• Collaborate with health care team to coordinate patient care.
• Provide patient education using appropriate teaching methods.
