Job summary

Signature HealthCARE of Norfolk is looking for a dedicated LPN/LVN to provide direct nursing care, including medication administration and wound care, while supervising nursing assistants and ensuring safety and dignity for residents. Strong documentation and collaboration with the interdisciplinary team are essential.

The position is a night shift (11pm–7am) in a 169-bed facility, offering growth opportunities, comprehensive benefits, and a supportive work environment for dedicated healthcare

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
