Job summary

Signature HealthCARE of Norfolk seeks a dedicated LPN to provide direct nursing care to residents during the night shift (11pm–7am). You will administer medications, assist with ADLs, supervise nursing assistants, monitor residents, and contribute to care plans within an interdisciplinary team.

Ideal candidates hold an active LPN/LVN license in Virginia, have at least one year of clinical nursing experience, and are comfortable with EHR documentation.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and basic documentation.
• Proficiency with Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor residents’ physical and emotional status and notify physician of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges.
