Job summary

Park Place Transitional Care & Rehab in Great Falls, MT, is seeking a full-time night-shift LPN/LVN to support a state-of-the-art unit. You will work on high-acuity care with ventilator/tracheostomy capabilities, supervising nursing assistants and ensuring safe patient care.

You will also handle narcotic counts, transcribe orders, and communicate with the Nurse Manager, DON, and care team to maintain quality outcomes in a family-friendly, on-site unit setting.

Qualifications

• Graduate of an approved practical/vocational nursing program.
• Current LPN/LVN license in good standing in MT or a compact state.

Responsibilities

• Direct and monitor nursing assistants on the unit.
• Follow procedures for narcotic counts, pharmacy orders and restraint reduction.
• Communicate regularly with the Nurse Manager or DON.
• Document and report changes in condition, including nutrition, infections, skin and weight.
• Give and receive shift report.
• Help admit and orient new residents; transcribe and communicate physician orders.
