Job summary

Putnam Nursing & Rehabilitation Center in Putnam County, New York, is seeking an LPN to join our newly led nursing team. Full-Time or Part-Time availability; you will provide direct resident care, monitor vitals, administer medications, and collaborate with the care team to update Nursing Care Plans.

Qualifications

• Graduate of an approved school of Nursing.
• Licensed and currently registered in New York as an LPN.
• Excellent communication, customer service, and organization skills.
• Willing to work under pressure and harmoniously with staff.
• Covid-19 vaccinations encouraged but not mandated.

Responsibilities

• Provide daily support to unit leadership and respond to resident/family needs.
• Use nursing process, judgment, and skills to set priorities for direct resident care.
• Administer standard nursing treatments and medications and monitor effects.
• Communicate and collaborate with care team to update Nursing Care Plan and findings.
• Monitor vitals, treatments, and residents’ responses.
• Oversee CNA assignments and activities to meet resident needs.
