Job summary

Putnam Nursing & Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) to join our newly renovated facility in Putnam County, NY. The LPN will support unit leadership, administer treatments and medications, monitor patient responses, and collaborate with the care team to maintain high-quality, compassionate resident care.

Full-time or part-time availability, with a focus on delivering safe, evidence-based nursing under NY requirements.

Qualifications

• Graduate of an approved school of Nursing.
• Licensed and currently registered in New York as an LPN.
• Commitment to compassionate, quality resident care.
• Ability to work under pressure and with a cooperative team.
• Vaccinations encouraged but not mandated; follow CDC guidance.

Responsibilities

• Provide daily support to unit leadership and respond to resident/family needs.
• Use nursing process, judgment, and skills to set priorities and deliver direct resident care.
• Administer standard nursing treatments and medications and monitor effects.
• Communicate with care team to revise/update Nursing Care Plans and findings.
• Monitor vitals, treatments, and residents’ responses.
• Organize CNA assignments and oversee delivery of resident care.

Licensed Practical Nurse, LPN Position Summary

• Licensed Practical Nurse, LPN, Full-Time or Part-Time Availability
• LPN provide daily support to the unit leadership & respond appropriately/timely to resident/family needs within the LPN scope of practice
• LPN will use nursing process, judgment, & skills to set priorities necessary to deliver quality direct resident care & make decisions promoting the safety/quality direct care to resident’s
• LPN perform/administer standard nursing treatments and/or medications & observes resident for desired or unwanted effects
• LPN communicates & collaborate with appropriate member(s) within the care team to revise/update the Nursing Care Plan or any pertinent findings including lab/test results, incidents, &/or concerns
• LPN monitors & documents vitals, treatments administered, & residents’ responses
• Prepare, organize, oversee, monitor, & evaluate delivery of care of resident CNA assignments & activities, considering the category of needs & competency of personnel

Requirements

• Graduate of an approved school of Nursing
• Licensed & currently registered in good standing in New York as an LPN
• Dedicated to providing extraordinary, clinically sound, & compassionate care
• Must possess a spirit of cooperation, caring & enthusiasm to create an atmosphere conducive to rehabilitation & optimal resident health
• Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents
• Have excellent communication, customer service, & organization skills
• Work well under pressure & willing to work harmoniously with all staff
• Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC
