Job summary

SSM Health Neuro Transitional Center, a joint venture with Select Medical, is seeking a Licensed Practical Nurse (LPN) to supervise Neuro Rehabilitation Coaches and deliver direct patient care in a 12-bed neuro rehab setting in O’Fallon, MO. Full-time night shift 7p-7a with rotation and weekend duties.

Compensation includes $34/hr PRN rate plus $4 night shift differential. The role focuses on coordinating patient care plans, staff supervision, and ensuring safe, quality rehabilitation.

Qualifications

• Licensed Practical Nurse (LPN) credential required
• Current State Driver’s License in good standing
• BLS certification required
• 3–5 years of experience in relevant setting

Responsibilities

• Supervises Neuro Rehabilitation Coaches on assigned shift and ensures adherence to treatment plans
• Educates patients, families and coworkers and maintains safety and wellbeing of all patients
• Coordinates staffing levels with NT Center Leadership and ensures adequate coverage
• Assists patients with meal planning, transportation and daily activities as part of rehab goals
• Performs nursing tasks such as vital signs, medication administration for up to 12 patients and other nurse delegation duties
