Job summary

Bayleylife is opening a new Assisted Living Memory Care Unit and seeks a dedicated LPN to provide clinical leadership and oversee daily resident care on day and evening shifts. The role blends nursing services with supervision of activity, housekeeping, nutrition, and resident programs to ensure safe, person-centered care.

The ideal candidate has an LPN license and strong dementia-care knowledge, with excellent communication and leadership skills to coordinate across departments.

Qualifications

• Licensed Practical Nurse program completed.
• Current Ohio LPN license in good standing.
• Ability to provide clinical leadership and staff supervision.
• Knowledge of person-centered dementia care principles.
• Ability to coordinate multiple departments within memory care.
• Strong communication, organizational, clinical judgement, and problem-solving skills.
• Compassion, patience, flexibility, accountability and leadership.
• Experience in assisted living or memory care preferred.

Responsibilities

• Provide nursing services per facility policies and resident care plans.
• Complete assessments and monitor changes in residents' status.
• Notify physicians and families of significant changes.
• Oversee medication administration and orders.
• Lead on-unit operations and supervise staff.
• Coordinate admissions, transfers, and discharges.
• Oversee Meaningful Moments program, dining, and environmental services.
• Coach staff in dementia-care techniques and non-pharmacological approaches.
