Job summary

Bayley Life is seeking a Licensed Practical Nurse to lead the clinical and daily operations of a new Assisted Living Memory Care unit. You will oversee resident care, the Meaningful Moments program, and collaborate with activities, housekeeping, nutrition, families, and leadership to ensure safe, compassionate, person-centered care.

The role requires LPN licensure in Ohio, clinical leadership, and dementia-care knowledge, with opportunities for both full-time and part-time schedules.

Qualifications

• Complete LPN curriculum from an accredited program.
• Current Ohio LPN license in good standing.
• Ability to provide clinical leadership and supervise staff.
• Knowledge of person-centered dementia care principles.
• Ability to coordinate multiple departments within memory care.
• Strong communication, organizational, clinical judgement, and problem-solving skills.
• Demonstrates compassion, patience, flexibility, accountability, and leadership.
• Experience in assisted living/geriatrics/memory care preferred; dementia care preferred.

Responsibilities

• Provide nursing services per Ohio Assisted Living/Residential Care Facility requirements and resident service plans.
• Complete nursing assessments and monitor changes in residents.
• Notify physicians, families, and others of significant changes.
• Ensure orders are implemented and medication management practices are followed.
• Provide treatments within the nurse’s scope of practice.
• Monitor residents after falls, incidents, or hospital visits.
• Coordinate admissions, transfers, discharges, and returns from hospital or other settings.
• Ensure resident service plans reflect current needs and interventions.
• Maintain timely and accurate documentation and participate in investigations as assigned.
• Report suspected abuse or neglect according to facility policy.
• Daily unit oversight and supervision of staff assignments.
• Promote teamwork and maintain a calm, resident-centered atmosphere.
• Oversee Meaningful Moments activity program, housekeeping, environmental, and nutrition services.
