Trinity Health Mid-Atlantic is hiring LPNs for Our Lady of Mercy Life Center’s Memory Care Unit in New York. Responsibilities include hands-on nursing care under RN supervision, medication administration, IV monitoring, and collaboration with an interdisciplinary team.

Qualifications

• Current New York State LPN license.
• Preferably 1 year of LPN experience in a skilled nursing setting.
• Pay range: $24.00 - $31.10; exempt positions paid within base range; vary by location.

Responsibilities

• Provide hands‑on nursing care under RN supervision.
• Assist with treatments, procedures, and resident care activities.
• Collaborate with the RN and interdisciplinary team to collect data and establish care priorities.
• Contribute to development and communication of individualized care plans.
• Administer medications and monitor IV therapy per established standards.
• Uphold nursing policies, procedures, and regulatory requirements.
• Offer compassionate support to residents and their families.
