Job summary

Trinity Health Mid-Atlantic is seeking a Full-Time Evening LPN for Our Lady of Mercy Life Center's Sub-Acute Rehabilitation unit. The role runs 3:00 PM to 11:00 PM, five shifts per week with every other weekend.

You will provide skilled nursing care, administer medications, and help residents regain strength and independence in a supportive setting. New York LPNs with a Practical Nursing program diploma are encouraged to apply.

Qualifications

• Current New York State Licensed Practical Nurse (LPN).
• Graduate of an accredited Practical Nursing program.
• Strong clinical assessment and communication skills.

Responsibilities

• Provide skilled nursing care in the Sub-Acute Rehabilitation unit.
• Administer medications and treatments per physician orders and nursing standards.
• Conduct resident assessments and report changes to RN Supervisor.
• Assist in developing, implementing, and updating individualized care plans.
• Educate residents and families about treatments, goals, and ongoing care needs.
• Collaborate with interdisciplinary team to support rehabilitation outcomes.
