Job summary

Livingston Regional Hospital is seeking an Licensed Practical Nurse (LPN) for Medical/Surgical units. The LPN delivers direct and indirect patient care under RN supervision, including vital signs, wound care, and ADL assistance.

You will ensure safety and adherence to care plans per hospital policy. Ideal candidates hold an accredited LPN program, TN licensure, and BLS certification, with six months of Med/Surg experience preferred.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current state LPN/LVN licensure required.
• Basic Life Support (BLS) certification as required by policy.
• De-escalation training as required by facility policy.
• Six months of Med/Surg experience preferred.

Responsibilities

• Provide direct patient care including vitals monitoring and ADLs assistance.
• Administer medications as ordered within scope of practice.
• Document care, assessments, and interventions promptly.
• Assist with specimen collection and wound care as directed.
• Collaborate with RN to develop and update care plans.
• Monitor patient condition and report changes to RN/physician.
• Educate patients and families about care plans and discharge.
• Maintain safe, clean environment following infection control.
• Participate in staff training, competency, and quality initiatives.
• Respond to emergencies and assist with patient stabilization.

Job Summary

The Licensed Practical Nurse (LPN) – Medical/Surgical provides direct and indirect patient care under the supervision of a Registered Nurse. The role includes monitoring vital signs, administering medications, performing wound care, and assisting with activities of daily living. The LPN ensures patient safety, adherence to care plans, and optimal outcomes in accordance with established nursing standards and hospital policies.

Essential Functions

• Provides direct patient care, including monitoring vital signs and assisting with ADLs
• Administers medications as prescribed within scope of practice
• Documents patient care, assessments, and interventions accurately and timely
• Assists with specimen collection, wound care, and other clinical procedures as directed
• Collaborates with the RN in developing, updating, and implementing patient care plans
• Monitors and reports changes in patient condition to the RN or physician
• Provides patient and family education regarding care plans and discharge instructions
• Maintains a safe and clean patient environment in accordance with infection control protocols
• Participates in staff training, competency assessments, and quality improvement initiatives
• Responds to emergencies and assists in patient stabilization as directed
