Job summary

Avita Health System in Ontario, OH, seeks a dedicated Licensed Practical Nurse (LPN) to join the MedSurg Department. You will gather data, plan and implement nursing care under RN supervision, ensure continuity of care between shifts, and support nursing assistants.

We’re currently seeking a dedicated Licensed Practical Nurse (LPN) to join our MedSurg Department at our Ontario location.

Position Overview

Gathers data, plans and implements the nursing care of patients within the nursing unit under the supervision of a RN. Responsible for ensuring continuity of care of the patients between shifts by providing direct care as well as assisting with the care given by nursing assistants and supportive staff members. Participates in patient and family teaching. Maintains role as patient advocate with a focus on the facility’s mission.

Qualifications

Required:

• Current licensure as a Licensed Practical Nurse in State of Ohio.
• Current BLS certification.

Preferred:

• Two (2) or more years of previous experience in rehabilitation nursing, longer term care or hospital setting.
