Job summary

Center for Human Development in West Springfield, MA is seeking a full-time Licensed Practical Nurse (LPN) for our Congregate Care Network. The LPN provides clinical oversight for medication administration across regional sites and supports safe, compassionate care for youth in DCF congregate care programs.

You will coach staff, review records, and coordinate with prescribers, pharmacies and community providers to ensure compliance with policy and licensing standards.

Qualifications

• Clinical oversight of medication administration across sites.
• Experience coaching and training healthcare staff.
• Familiarity with medication policies and regulatory requirements.

Responsibilities

• Review medication practices for compliance with orders and MAP policies.
• Verify medications are ordered, stored, documented and administered correctly.
• Monitor vital signs, glucose, allergies and prescriber notifications.
• Observe and document annual competency observations for staff.
• Provide coaching and in-service education with RN partnership.
• Review MORs and coordinate with program leaders on corrective actions.
• Support medication reorders, reviews and prescriber paperwork.
• Maintain medication rooms and communicate deficiencies to leadership.
