Job summary

Sunrise Senior Living seeks a licensed practical nurse (LPN) to administer medications and monitor residents in Setauket, NY. You will follow physician orders, document in MAR/TAR in real time, and report changes promptly to ensure safety and quality of care.

Required: Active LPN license in good standing, CPR and First Aid certification, strong assessment and documentation skills; prior senior care or medication administration experience is preferred.

• Conduct Service & Health Updates (SHUs).
• Transcribe physician orders and support care plan updates.
• Observe and report physical, emotional, or cognitive changes.
• Maintain medication carts, complete narcotic counts, and uphold compliance standards.
