Job summary

Minnesota Adult & Teen Challenge is seeking a Medication Coordinator (LPN) in Rochester, MN. The role supports nursing operations under RN supervision, coordinating medication management, reconciliation, and provider communication to improve treatment outcomes.

Responsibilities include medication ordering, inventory, client education, and ensuring compliance with DHS/MDH regulations and MnTC policies. Requires an active MN LPN license and strong communication skills.

Qualifications

• Minnesota LPN license in good standing.
• Graduate of an accredited practical nursing program.
• 1 year nursing experience preferred; behavioral health/substance use background preferred.

Responsibilities

• Review and reconcile medications upon admission/transfer/discharge.
• Coordinate medication ordering, refills, and renewals with pharmacies and clinicians.
• Monitor medication inventories and assist with supplies.
• Educate clients about medications, side effects, and adherence.
• Maintain accurate documentation in EHR and liaise with treating providers.
