Job summary

LiveWell Homecare Agency, LLC is seeking an LPN Medication Nurse to provide licensed nursing support in senior care settings, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

You will administer medications per physician orders, document in MAR/TAR in real time, monitor residents for adverse reactions, maintain carts and narcotic counts, and report health changes promptly to support care plans and regulatory compliance.

Qualifications

• CPR and First Aid certification required.
• Strong assessment, documentation, and observation skills.
• Preferred experience in senior care or medication administration.

Responsibilities

• Document accurately in MAR/TAR in real-time and transcribe physician orders to support care plan updates.
• Monitor residents for symptoms, adverse reactions, and physical, emotional, or cognitive changes.
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulatory standards.
• Report refusals, reactions, and health changes promptly.
• Conduct Service & Health Updates (SHUs).
