Job Overview

The LPN Medication Care Manager provides quality care by administering medications and treatments safely, documenting accurately, and supporting residents in maintaining their dignity and independence, in accordance with laws, regulations, and Sunrise policies.

Responsibilities & Qualifications

Responsibilities:

Supporting our Mission, Principles of Service, and Core Values, the role includes but is not limited to:

• Medication Administration: Safely administer and document medications, review medication updates, assist with medication refills, and maintain confidentiality and safety standards.
• Resident Care: Communicate concerns, participate in care planning, and support residents’ independence and dignity.
• Risk Management & Safety: Ensure compliance with safety regulations, report incidents, and handle hazardous materials properly.
• Team Contribution & Training: Participate in team meetings, training, and may supervise other team members.

Qualifications:

• Licensed Nurse (LPN, LVN, or equivalent).
• Valid certifications, including CPR and First Aid.
• At least 18 years old; experience with seniors preferred.
• Strong communication, judgment, and decision-making skills.
