Job summary

Citadel Healthcare in Skokie, IL is seeking a Licensed Practical Nurse (LPN) to care for residents, implement plan of care, and collaborate with the IDT to deliver quality nursing services.

You will perform assessments, monitor vital signs, administer treatments within licensure, document care, and ensure safety and infection-control compliance while maintaining HIPAA standards. Full-time, on-site role with opportunities for growth and comprehensive benefits.

Qualifications

• Nursing degree or diploma from an accredited college or university.
• Current license to practice in the State as an LPN.
• 1 year of experience preferred; long-term care experience preferred.
• Excellent written and verbal communication in English.
• Able to adapt to a flexible schedule and occasional overnight travel.

Responsibilities

• Provide licensed care to residents as ordered by physician and in accordance with standards.
• Perform resident assessments and review care plans for appropriate care.
• Monitor vital signs, administer IV therapy and respiratory care as permitted by license.
• Document nurse notes reflecting care provided and resident responses.
• Maintain a safe environment and address potential safety risks promptly.
