The position requires Missouri-licensed LPN, at least three years direct care experience including LTC, and proven ability to develop and maintain care plans and MDS records. Collaboration with Nursing leadership ensures resident quality of life.

Qualifications

• Must be a Missouri-licensed practical nurse.
• At least three years direct care experience; LTC experience preferred.
• Ability to assess patient needs and develop comprehensive care plans and MDS records.
• Knowledge of HFCA regulations, PPS, and RUGS standards.

Responsibilities

• Manage MDS coordination and care planning for LTC residents.
• Ensure compliance with HFCA/state regulations and guidance.
• Maintain accurate Care Plans, MDS records, charts, and reports.
