Job summary

Agawam West Rehab & Nursing seeks an experienced LPN to serve as MDS Coordinator on a full-time, salaried basis. You will oversee resident assessments, ensure accurate clinical documentation, and coordinate MDS submissions in collaboration with nursing, therapy and the interdisciplinary team.

The position offers competitive salary ($91,000–$95,000/year DOE), a Quality Measure Bonus up to $7,500/year, census-based increases, and comprehensive benefits (medical, dental, vision, 401(k), PTO).

Qualifications

• Active Massachusetts LPN license required.
• MDS experience in a skilled nursing facility required.
• Strong understanding of the MDS process, including coordination, scheduling, and completion.
• Long-term care or rehabilitation experience.
• Strong organizational and attention-to-detail skills.

Responsibilities

• Coordinate, schedule, and complete MDS assessments
• Collect and review clinical information from nursing, therapy, and medical records
• Ensure accurate and timely documentation and submissions
• Participate in interdisciplinary care planning and resident assessment reviews
• Support regulatory compliance and accurate reimbursement
• Maintain knowledge of current MDS and CMS requirements

Education

Massachusetts LPN license

MDS experience in a skilled nursing facility

Strong understanding of the MDS process

Long-term care or rehabilitation experience

Strong organizational and attention-to-detail skills
