Job summary

Distinctive Living in Maumee is seeking a full-time LPN/LVN to assess resident health and develop care plans under the Director of Health and Wellness. You'll coordinate with nursing staff, ensure regulatory compliance, and contribute to ongoing CQI activities.

The role requires a valid LPN/LVN license, at least one year of nursing or caregiver experience in assisted living/memory care, and supervisory exposure. Strong communication and teamwork are essential for success.

Qualifications

• High School Diploma or equivalent required; LPN/LVN licensure required.
• One year of nursing or caregiver experience, preferably in assisted living, home health or memory care.
• Minimum six months supervisory experience.
• Experience with Microsoft Word and Excel preferred.
• Must read physician orders and match to MARs.
• Understanding of medical terminology sufficient for communicating with medical professionals.
• Excellent verbal and written communication skills.

Responsibilities

• Follows resident service plans and documents changes of condition in accordance with regulations.
• Communicates needs to Director, Health and Wellness to ensure consistent quality services.
• Provides hands-on care and nursing tasks in support of the resident's care plan.
• Provides clinical support and training to care staff.
• Transcribes physicians' orders and reviews new orders.
• Maintains required certifications and completes ongoing in-service trainings.
• Ensures compliance with CQI program and reports issues to leadership.
• Reviews MARs and Daily Reports to ensure appropriate interventions.
• Adheres to HIPAA, OHSA, and Blood Borne Pathogen guidelines.
• Works safely and supports others in safety initiatives.
• Models professional behavior with residents, families, and staff.
• Performs other duties as assigned.
