Job summary

Distinctive Living is seeking an LPN/LVN (Full-Time) to join our Maumee Pointe Assisted Living and Memory Care team. You will assess resident health, develop and update care plans, and coordinate with the Director of Health and Wellness to ensure regulatory compliance and high-quality care.

The ideal candidate has a valid LPN/LVN license, at least one year of nursing or caregiving experience in assisted living or memory care, and strong communication skills.

Qualifications

• LPN/LVN licensure required.
• 1+ year of nursing or caregiver experience in assisted living, home health, or memory care preferred.
• Minimum six months supervisory experience.

Responsibilities

• Follows resident service plans and documents changes per local regulations.
• Communicates needs to Director of Health and Wellness to ensure consistent quality services.
• Provides hands-on care and supports the resident's care plan in line with state regulations.
• Trains staff and reviews medication administration and orders as required.
• Maintains certifications and completes in-service trainings; supports CQI processes.
