Job summary

Distinctive Living in Maumee Pointe Assisted Living and Memory Care is seeking a full-time LPN/LVN to assess resident health status, develop care plans, and liaise with healthcare staff. You’ll ensure regulatory compliance, review MARs, and train care staff to uphold quality standards.

The ideal candidate holds an LPN/LVN license, with at least one year of nursing or caregiving experience in a related setting, plus supervisory exposure. Strong communication and documentation skills are essential.

Qualifications

• High School Diploma or equivalent required; LPN/LVN licensure required.
• Minimum one year nursing or caregiver experience in assisted living, home health, or memory care.
• Minimum six months supervisory experience.
• Experience with Microsoft Word and Excel preferred.
• Ability to read physician orders and match to Medication Administration Records (MARs).
• Understanding of medical terminology sufficient for communicating with medical professionals.
• Excellent verbal and written communication skills.
• Able to communicate verbally in person and over the phone.

Responsibilities

• Follows the services outlined in each resident's service plan and documents changes as required.
• Communicates resident needs to the Director of Health and Wellness to ensure consistent quality services.
• Performs hands-on care and nursing tasks in support of the resident's care plan and verifies plan alignment with state regulations.
• Provides clinical support and training to care staff for ongoing education requirements.
• Transcribes physicians' orders and reviews new orders for accuracy and appropriateness.
• Maintains required certifications and completes in-service trainings and licensure requirements.
• Supports CQI processes and ensures compliance with HIPAA, OSHA, and Bloodborne Pathogen guidelines.
• Promotes safety, teamwork, professionalism, and accurate documentation in all interactions.
