Qualifications

• High School Diploma or equivalent required.
• LPN/LVN licensure required.
• One year of nursing or caregiver experience, preferably in assisted living, home health or memory care.
• Minimum six months supervisory experience.
• Experience with Microsoft Word and Excel preferred.
• Must read physician orders and match to MARs.
• Understanding of medical terminology to communicate with medical professionals.
• Excellent verbal and written communication skills.
• Able to communicate verbally in person and over the phone.

Responsibilities

• Follows resident service plans in a friendly, supportive manner.
• Communicates with Health and Wellness Director on resident needs.
• Provides hands-on care and nursing tasks per care plan; reviews plans for regulatory compliance.
• Provides clinical support and training to care staff.
• Assists in transcription of physicians' orders and reviews new orders.
• Maintains required certifications and ongoing in-service training.
• Supports CQI program and reports quality issues to leadership.
• Reviews MARs and Daily Reports for appropriate clinical interventions.
