Job summary

Resolution Think LLC. seeks a Licensed Practical/Vocational Nurse (LPN/LVN) to join our Referral Management Center team at a Military Treatment Facility.

You will support referral management, clinical-administrative review, documentation, and patient education, tracking referrals and ensuring timely completion.

Requirements include US citizenship, active LPN/LVN license, one year of nursing experience, BLS, and strong English skills.

Qualifications

• Must be licensed as an LPN/LVN in the US and maintain current licensure.
• One year of practical or vocational nursing experience required; referral management preferred.
• Basic computer skills and ability to navigate electronic health records.
• Must read, write, speak, and understand English fluently.
• Ability to obtain a favorable background check and necessary facility badges.

Responsibilities

• Track and follow up on referral reports and documentation.
• Ensure referrals from network/VA providers are returned within required timeframes.
• Review referrals requiring nursing judgment and maintain accurate statuses.
• Support specialty referral and recapture initiatives.
• Educate patients and MTF staff on referral requirements and timelines.
