Job summary

Chandler Memory Care is seeking a licensed practical nurse (LPN/LVN) for Cardinal Landing Memory Care in Bowling Green, KY. This full-time night shift role emphasizes patient-centered care, medication administration, and collaboration with physicians and CNAs to support residents.

You will deliver compassionate clinical care, maintain accurate records, participate in admissions and transfers, and guide residents and families on health management and discharge instructions.

Qualifications

• Current LPN/LVN license in Kentucky
• Strong clinical and patient management skills
• Excellent communication and interpersonal skills
• Basic Life Support (BLS) certification required; ACLS preferred
• Ability to lift, push, or pull 50 pounds and bend, twist, stoop, kneel, and reach and sit for long periods

Responsibilities

• Administer medications, treatments, and wound care as prescribed by physicians.
• Monitor and assess patient conditions and promptly report changes.
• Assist in patient admissions, discharges, and transfers.
• Provide compassionate care, ensuring patients' comfort and dignity.
• Maintain accurate patient records and documentation in accordance with facility policies.
• Coordinate with doctors, CNAs, and other healthcare team members to deliver comprehensive care.
• Educate patients and families on health management and discharge instructions.
