Job summary

Chandler Memory Care is seeking a compassionate LPN/LVN to provide patient-centered care at our Bowling Green, KY location. You will administer medications, monitor conditions, and collaborate with a multidisciplinary team to ensure safe, dignified care for residents.

This part-time/PRN position offers On-Demand Pay and a supportive environment with growth opportunities in a memory care setting.

Qualifications

• Current LPN/LVN license in the state
• Strong clinical and patient management skills
• Excellent communication and interpersonal skills
• Ability to work in a fast-paced environment

Responsibilities

• Administer medications, treatments and wound care as prescribed.
• Monitor patient conditions and report changes promptly.
• Assist admissions, discharges and transfers.
• Maintain accurate patient records and documentation.
• Coordinate with doctors, CNAs and other staff to deliver care.
• Educate patients and families on health management and discharge instructions.
