Job summary

Calvert-Home-Health-and-Hospice in Lubbock, TX is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide in-home nursing services under the direction of the RN Case Manager.

This PRN opportunity offers flexible scheduling, driving to patient homes, and participation in ongoing training; licensure and driving eligibility are required, with benefits for full-time staff.

Qualifications

• Licensed as LPN/LVN through state Board of Nursing.
• Preferred 1 year in home health, medical, surgical, or critical care.
• Must be 18+ and physically/mentally fit.
• Pass pre-employment background and health screening.
• Valid driver with insured vehicle in good working order.
• Self-directed with minimal supervision.
• Excellent observation, clinical judgement, and communication.
• Demonstrates flexibility and cooperation in fulfilling role obligations.

Responsibilities

• Provide safe home health services and education to patients, families, and caregivers.
• Carry out plan of care initiated by the RN Case Manager with the physician.
• Develop, prepare, and maintain individualized patient care documentation.
• Coordinate care with patient, caregivers, other clinicians, and physician.
• Participate in company meetings and in-services.

About the Position

TheLicensed Practical Nurse LPN/Licensed Vocational Nurse LVN furnishes services in accordance with agency policies, prepares clinical and progress notes, assists the physician and registered nurse in performing specialized procedures, prepares equipment and materials for treatments observing aseptic technique as required; and assists the patient in learning appropriate self-care techniques.

• Shows excellent observation, good clinical judgement, and good oral and written communication.
