Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Job Summary

Wheeler Staffing Partners is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join a Home Health team. This role is responsible for delivering high-quality, patient-centered care in patients’ homes. The LVN will collaborate closely with an interdisciplinary healthcare team to promote wellness, manage chronic conditions, and support optimal health outcomes.

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
• Conduct routine nursing visits and assessments
• Monitor patient conditions and report changes to the care team
• Administer medications and treatments as prescribed
• Educate patients and caregivers on disease management and care plans
• Maintain accurate, timely, and compliant medical documentation
• Collaborate with physicians, therapists, and other healthcare professionals
• Ensure adherence to all clinical and regulatory standards

Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of Texas
• Minimum of one (1) year of clinical experience
• Minimum of one (1) year of home health experience
• Ability to perform nursing visits and maintain accurate medical records
• Strong communication, organizational, and patient care skills

Required Certifications & Licenses

• Graduate of an accredited practical or vocational nursing program
• Current CPR certification (must be maintained throughout employment)
• Valid driver’s license and reliable transportation with the ability to travel to patient locations
