Job summary

Crest Home Health and Palliative Care is seeking a reliable LVN/LPN to provide direct patient care, administer medications, perform treatments, and document on the patient’s records under the direction of the Case Manager. Most work travels to patients’ residences in the Boerne area.

Texas LVN/LPN license with current CPR is required; 1 year of clinical experience is preferred; flexible schedule and PRN/fulltime positions are available; home health experience preferred.

Qualifications

• Graduate of an approved LVN/LPN program.
• Current Texas LVN/LPN license.
• Current CPR; BLS certification preferred.
• 1 year clinical experience; home health experience preferred.

Responsibilities

Job Description:

We are looking for a reliable LVN/LPN (Licensed Practical Nurse/Licensed Vocational) to administer direct patient care, administer approved medications, perform treatments, make accurate nursing observation, and chart on the patients' medical record in a professional and considerate manner. Duties are performed under the direction of the Case Manager and/or Patient Care Manager. LVN/LPN professionals must be first and foremost qualified and compassionate. The ideal candidate preference will have at least one year experience as a nurse in a health care setting. Home health experience preferred. We offer competitive pay and flexible schedule.

Educational Requirements:

• Graduate of an approved program of vocational/practical nursing

Licensing/Certification/Registration Requirements:

• Current Licensed Vocational/Practical Nurse (LVN/LPN) from the Texas Vocational Nurse Examiners
• Current CPR
• BLS certification preferred.

Responsibilities:

• Assists in assessing patient and family deeds through the recognition and reporting of signs and symptoms and nursing observation to the assigned Case Manager
  • assists in assessing the health status of assigned patients.
  • complies with the established nursing assessment guidelines/policies.
• Utilizes the written plan of care for implementing nursing interventions necessary to accomplish the established goals.
• Complies with general performance standards of licensed vocational nurse. Demonstrates flexibility in response to unexpected changes in work volume, emergencies, staffing, or scheduling changes.
• Must have a reliable means of transportation, proof of financial responsibility (car insurance and a valid state driver’s license)
