Job summary

Cynet Health is seeking an LPN/LVN to provide nursing care in patients' homes within a 75-mile radius. Coordinate care, document per agency policies, and report to the Clinical Manager. Must have a current license and CPR/BLS, with at least 1 year of adult home health or hospice experience.

You will supervise Home Health Aides, travel daily, and complete documentation within 24 hours. Proficiency with HomeCare HomeBase EMR and reliable transportation are required.

Qualifications

Description:Provides coordination and direction for home nursing care according to physicians' orders.Utilizes agency policies and procedures.Applies the nursing process competently in patient care.Reports to the Clinical Manager.Supervises and evaluates Home Health Aides and Licensed Practical Nurses.Responsible for traveling to patient homes and attending office meetings.Must have reliable transportation and a current driver's license.Agency-required liability insurance is necessary.Traveler will handle between 5-10 patients per day.Documentation and travel times are accounted for in productivity points.Types of cases include Start of Care, Wound Care, and Follow-up Visits.Must be comfortable with a travel radius of up to 75 miles.Mileage will be reimbursed at the IRS standard rate.All documentation must be completed within 24 hours of patient appointments.No guaranteed hours; meet weekly productivity points requirements.Declining patients may result in contract cancellation.Must accept all assigned patients regardless of distance from previous appointments.Proficient in HomeCare HomeBase EMR required.Must have used the system within the last 6 months.
