Job summary

• No current direct employment with any home health location when applying as a traveler
• Ability to travel up to 75 miles from assigned care center

Description:Provides coordinated and directed home nursing care according to physicians' orders.Care is based on agency policies and procedures through the competent application of the nursing process.Responsible to the Clinical Manager.Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.

Home visits required, along with periodic office meetings.Must have reliable transportation and current driver's license.Agency-required liability insurance is necessary.Average number of patients per day: 5-8.Productivity points per week: 30 points.Each visit and activity holds a different point value.Contractor will be assigned 5-10 patients per day that equate to roughly 6-8 points.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.All documentation must be completed within 24 hours of the patient appointment.Delayed or incomplete documentation impacting billing and follow-up patient care may result in cancellation.Documentation standards must be met consistently.

Traveler must be capable of accepting every patient on their schedule, regardless of location.No guaranteed hours with this contract.Weekly productivity points must be met to maintain position.If not receiving enough patients each week to meet financial needs, contractors may be floated to a nearby clinic to pick up patients.

Comfortable with drives that may exceed 20 minutes between patients is required.Local candidates are accepted at the same rate.All candidates must understand expectations and must be prepared to travel as needed.
