Job summary
Sevita in Bedford, Indiana is seeking an experienced Licensed Practical Nurse / Licensed Vocational Nurse to deliver nursing services and direct support within our residential programs. You will monitor health conditions, administer medications, and ensure coordinated care for individuals with complex medical and behavioral needs.
You will train and supervise staff, conduct health assessments, and collaborate with physicians and therapists to implement ISP-based plans.
Qualifications
- Association degree in nursing and active LPN/LVN license for residence state.
- At least one year of nursing experience with the target population.
- Ability to effectively communicate with staff, individuals served, and families.
Responsibilities
- Monitor health conditions of individuals served and ensure quality services.
- Administer medications and document, oversee staff who medicate.
- Provide medical treatment per physician orders and personal care.
- Conduct assessments and update ISP-based health care services.
- Train staff on precautions, infection control, and medication administration.
- Liaise with medical providers and external agencies to coordinate care.
- Provide training on self-medication, treatments, and wellness topics.
