Job summary

Sevita is seeking a Licensed Practical Nurse/Licensed Vocational Nurse to join our ResCare Community Living team in Toledo, Ohio. You will provide nursing services, administer medications, and help individuals with developmental and behavioral needs live more independently.

You will assess health conditions, coordinate ISP plans, train staff, and liaise with medical providers to ensure consistent, person-centered care.

Qualifications

Associate’s degree in nursing or related field and valid licensure.
At least 1 year nursing experience with the served population.
Current LPN/LVN licensure for residence state and valid driver’s license.
CPR/FA, HIV/AIDS, BBP and ACT training as required by state regs.

Responsibilities

Monitor health conditions and ensure quality services.
Administer medications and document care; supervise staff administering meds.
Provide physician-ordered treatments and personal care services.
Assess needs, implement ISP health care steps, adjust as conditions change.
Train staff on precautions, infection control, and med administration.
Coordinate with medical providers and external agencies for consistent care.
Educate individuals on self-care topics including medications and sex education.
