Job summary

Heart to Heart Hospice seeks an on-call LVN/LPN to support patient care at home under an RN directive. You will perform home visits, observe and report findings, assist with the Plan of Care, and help patients learn self-care techniques. Compliance with policies and timely documentation is essential.

The role emphasizes end-of-life care support, patient comfort, and collaborative care within a compassionate hospice team.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current State LVN/LPN license preferred.
• Two years of LVN/LPN experience in clinical care preferred, hospice/home health preferred.
• Current State Driver's License and reliable transportation with valid auto liability insurance.

Responsibilities

• Under On Call RN/Triage RN direction, support patient nursing needs per Plan of Care.
• Provide home visits as directed to observe and report findings to the RN.
• Assist the RN in carrying out the Plan of Care.
• Help patient learn appropriate self-care techniques.
• Work in accordance with policies and regulations defining service scope.
• Assist RN/physician with end-of-life care duties.
• Submit documentation promptly.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.

What You Must Have

• Graduate of an accredited school of vocational nursing
• Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse preferred
• Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting preferred, hospice or home health preferred
• Current State Drivers License
• Must have reliable transportation and provide ongoing valid and current auto liability insurance

What You Will Do

• Under the direction of the On Call RN/Triage RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care (POC)
• Provide home visits as directed by the On Call RN/Triage RN to observe and report findings to the appropriate RN.
• Assist the RN in caring out the POC
• Assist the patient in learning appropriate self-care techniques
• Provide services in accordance with organizational policies and procedures and regulations that define the scope of services
• Assist the RN and/or physician in performing specialized duties related to end-of-life care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
