Sarah Bush Lincoln is seeking an LPN for long-term care, providing direct patient care on a 6pm–6am shift with weekend/holiday rotation. The role includes pain management, medication administration, comprehensive documentation, and ongoing education for residents and families.

You will coordinate with the care team, assist with ADLs, and participate in discharge planning while maintaining quality and safety standards across shifts.

Qualifications

• Pain assessment and management.
• Accurate documentation of care and procedures.
• Effective communication with residents, families, and team members.
• Evidence-based nursing actions according to protocols.

Responsabilités

• Assess and reassess pain; educate residents/families on pain management.
• Administer medications and treatments per physician orders; review orders.
• Assess residents on admission and quarterly; document changes and MDS.
• Coordinate discharge planning with the care team.
• Document care thoroughly and reflect patient condition changes.
• Lead and participate in daily care; delegate tasks and provide feedback.
• Notify physicians of condition changes and document actions taken.
• Provide ADL assistance and general patient care.
• Collaborate to modify care plans with appropriate disciplines.
• Respond promptly to call lights and patient needs.
• Educate patients and families about plans of care.
