Job summary

Reliable Healthcare Associates, LLC is seeking a Licensed Practical Nurse (LPN) to provide planning and delivery of patient care at client facilities across Wisconsin. We offer premium flat-rate pay and flexible scheduling with plenty of hours available.

You will plan, deliver and document patient care, supervise nursing assistants, and administer medications per physician orders, while maintaining professional communication with residents, families, and staff.

Qualifications

• Graduate or future graduate from an accredited nursing program.
• Hold a Current State License as a Licensed Practical Nurse.
• Minimum 1 year LPN experience in long-term care preferred.
• Willing to pick up shifts at facilities located in Wisconsin.

Responsibilities

• Provide planning and delivery of direct and indirect patient care through the nursing process (assessment, planning, prevention, evaluation).
• Direct the day-to-day activities of the nursing assistants.
• Prepare and administer medications as ordered by physician and perform other drug administration.

• Complete required record keeping upon residents’ admission, discharge, transfer; chart and document per facility policy.

• Hold a Current State License as a Licensed Practical Nurse
• Able to communicate well in both verbal and written forms
• Have a minimum of 1-year experience as an L.P.N. in a long term care setting (preferred)
• A natural problem solver
• Are committed to interacting professionally and with care toward residents, families, and other staff
