Qualifications

Responsibilities

• Provide direct nursing care to residents in a long-term care setting.
• Administer medications and treatments according to physician orders and facility protocols.
• Manage approximately 15 patients per medication pass, with aides assisting.
• Assess residents and monitor changes in condition.
• Report significant changes to the healthcare provider.
• Educate patients on medications, treatments, and care plans.
• Guide and support LNAs/Nurse Aides.
• Collaborate with medication aides, nurses, physicians and the care team.
• Assist with development and implementation of care plans.
• Maintain accurate patient documentation (e.g., PCC).
• Follow infection-control, safety, and facility policies.
• Promote resident dignity, safety, and independence.
• Support quality patient care within staffing ratios.

Job Summary:

Tryfacta is seeking an LPN - LTC for our client in Berlin, NH 03570. This is a 13-week contract assignment.

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to residents in a long-term care setting.


• Administer medications and treatments according to physician orders and facility protocols.


• Manage approximately 15 patients per medication pass , with Medication Aides assisting with certain medications.


• Assess residents and monitor for changes in physical, emotional, and clinical condition.


• Report significant changes in resident condition to the appropriate healthcare provider.


• Provide patient education regarding medications, treatments, health conditions, and care plans.


• Provide guidance, education, and support to LNAs/Nurse Aides.


• Collaborate with Medication Aides, Nurse Aides, RNs, physicians, and other members of the healthcare team.


• Assist with development and implementation of individualized care plans.


• Maintain accurate and timely patient documentation using PointClickCare (PCC) .


• Follow infection-control, safety, and facility policies.


• Promote resident dignity, comfort, safety, and independence.


• Support quality patient care while maintaining a strong bedside presence.


• Work effectively within established staffing ratios and assist with unit needs as required.
