Job summary

Tryfacta, Inc. seeks a licensed practical nurse (LPN) with long-term care experience for a 13-week assignment in Hood River, Oregon. The role requires a valid LPN license and BLS certification, with a minimum of one year of nursing experience.

The LPN will support residents with daily care, administer medications, monitor health conditions, and collaborate with CNAs, RNs, and physicians while following safety protocols. The day shift runs Tue/Wed/Thu, 6am–6pm, with competitive hourly pay.

Qualifications

• 1+ year nursing experience preferred.

Responsibilities

• Administered medications and treatments as ordered.
• Monitored vital signs and residents' health conditions.
• Provided wound care and routine nursing procedures.
• Assisted residents with ADLs and personal care.
• Documented assessments, care, and medication administration.
• Reported changes in condition to the RN/provider.
• Followed infection control and patient safety protocols.
• Collaborated with CNAs, RNs, physicians, and other healthcare staff.

Job Summary:

Tryfacta is seeking a LPN LTC for our client in Hood River, OR 97031 . This is a 13 Weeks contract assignment.

Position Title: LPN LTC

Location: Hood River, OR 97031

Duration: 13 Weeks

Work Schedule: Day 3x12, 6am-6pm Tues/Wed/Thu

Pay rate: $ 45 to $51 per hour

Responsibilities for this position include, but are not limited to:
