Job summary

Osceola Regional Health Center is seeking an experienced Licensed Practical Nurse (LPN) for long-term care to provide direct nursing care and support the care plan within a collaborative team.

The role requires an Iowa LPN license, CPR within 3 months, and the ability to coordinate resident care with the RN and other disciplines. Regular daytime hours with rotating weekends and holidays are typical.

Qualifications

• Graduate of an accredited school of nursing.
• A minimum of two years of nursing and/or long-term care experience preferred.
• Current LPN license in the State of Iowa.
• Must obtain CPR certification within 3 months of hire date.
• State of Iowa approved child/dependent adult abuse mandatory reporter training within 6 months of hire.

Responsibilities

• Assist RN in documenting and actualizing the nursing process to provide patient care.
• Assist in performing and documenting initial and follow-up assessments as directed.
• Contribute to the development of the resident plan of care and implement nursing strategies.
• Coordinate care and supervise unlicensed personnel as needed.
• Collaborate with the multidisciplinary team to promote appropriate utilization of facility services.
